We do not keep records of provider verification requests. Your privacy comes first.

If a provider isn’t in our network yet, they can join for free.

Applications may take up to 48 hours to complete. You must be at least 25 years old to become a member. Expeditied service is available for an additional $25 charge during normal business hours.
